What’s the role all about ?

This role will support our flourishing partnership with Bexley, Bromley, Greenwich, Lambeth, Lewisham.

NHS England recently identified that Virtual Wards are a high impact intervention that can support the NHS this winter.

We are developing an Inreach service to bolster NHS@Home service over the winter and beyond.

Based in Bromley & Lewisham, you will work within the multidisciplinary NHS@Home team to increase the number of patients referred to the virtual ward service.

The role

This is a clinical role, working as an important lynchpin between referring medics in the respective hositals NHS@Home service.

You will lend your clinical expertise to facilitate the referral process so that more patients can get home to their loved ones more quickly.

Tasks will include:

  • Participate in case finding to identify suitable candidates for remote monitoring.
  • Undertake initial referral assessments with patients.
  • Support referring clinicians to finalise treatment plans quickly and effectively.
  • Liaise with the NHS@Home inreach coordinator to understand daily community-based capacity
  • Advocate the service to referring clinicians to increase their understanding and endorsement of NHS@Home’s capabilities.
  • Identify opportunities to diversify the departments who refer to the service.

About you


  • Be a registered Band 7 clinician (adult trained) with an active NMC pin

Desirable qualifications

  • Masters degree in a related field

Desirable experience:

  • working in a digital start up
  • working with remote patient monitoring


  • be able to read, write and speak English fluently;
  • have solid technology acumen;
  • have excellent written and verbal communication skills and well-developed interpersonal skills;
  • be excellent at team working with the ability to work using their own initiative;
  • be professional and patient-focussed;
  • demonstrate the clinical knowledge and awareness of the complexities of remote patient monitoring;
  • be accountable for their own actions in accordance with the Code of Professional Conduct;
  • be able to work autonomously with minimal direction;
  • have well-developed political awareness, influencing, negotiating and conflict resolution skills;
  • be able to motivate and inspire the clinical team to achieve quality patient outcomes;
  • be confident using IT systems MS office (including word and excel)

This is an onsite role working five days a week in either Bromley or Lewisham.
